Creating a form in Front Page

I'm having some problems creating a form in front page 2003. Our website is
hosted through powweb (which is a mistake in itself). Front Page Server
extensions are turned on, however when I create the form I've had more issues
uploading via ftp (as it's not supported with forms). I'm trying to figure
out (I have the form page now showing) but when you fill out the form and
click submit, it has a page come up saying that the confirmation page is a
member only page (have to be signed in). Which is not the case. Nor is it
sending me the email with form results. Any ideas?

If the FP extensions are installed, then you always need to open or publish your site via FP's http
mode, never via FTP.

Most likely you have corrupted the extensions and they will need to be repaired or reinstalled by
your web host.
