How do you insert check mark boxes in templates for Word 2007?

I am making a template in Word 2007. I can't figure out how to insert boxes
to check mark.

First, click Office button > Word Options > Popular and check the box
"Show Developer tab in the ribbon". Click OK.

Click the Developer tab in the ribbon. In the Controls group, click
the Legacy Tools button. The check box is in the Legacy Forms row of
the gallery.

To make the check box (and other form fields in that group) operate,
you need to click the Protect Document button on the Developer ribbon
(it's also on the Review ribbon) and choose to protect for filling in
forms. This may not be what you intended, as it prevents changes
anywhere in the document except in the form fields.
