How do you create a phone tree for a team or club?

I am in need of a template or easy way to create a phone tree for our
Neighborhood Watch program. There would need to be room for name and phone #
for about 20 contacts.

I would use a table for this. Apply borders selectively to create the tree
appearance.

You could also use a PowerPoint organization chart that would flow nicely in
a tree fashion.
