How do I set up letter templates with markers for changing info?

I'm a physician and I write letters to referring doctors about their
patient's conditions, etc. Many of the letters are repetitive (because I make
certain diagnoses over and over again). I want to set up a letter template
for each diagnosis with fields that I can tab between to enter a patient's
name, age, specific symptoms, etc. (Better yet I'd like to use my Philips
SpeechMike Pro to tab then type the information as I dictate it.)

How do I set up a letter so I can do what I want? I think it should look
like this:
Dear Dr. <<|>>,
I had the pleasure to see your patient <<|>> on <<Date>> whom you referred
for evaluation of <<symptom>>.
Blah blah blah,
Thank you again for referring <<patient>>.
