how do i put lines in a form, that others can type on?

I need to make an application form.
I want people to be able to type their answers on the lines on the form.
I've been stumped trying to figure out templates? I have found the drawing
toolbar to draw lines, but that seems not the most efficient was to line a
page. Also, how to keep them consistently the same length and distance apart.
Please help!

methods there, I'd recommend a table with selectively displayed borders.
