How do i print on top of a bitmap in Word?

I have a file in Word 2000 which (on the screen) shows a bunch of entries
which are fill in the blanks for a bitmap file already on the page. In
effect, it shows a credit card form with the details filled in.
When I print the file, the fill-ins do not show up.

This used to work fine, including the print, but Word shut itself down just
now, and when I rebooted, and tried to print, I got a blank credit card form.

Can anyone help?

On the Print tab of Tools | Options, make sure that "Drawing objects" is
checked.

Thanks for the suggestion, but "Drawing Objects" is checked, and the fill-ins
still don't print.
