how do i make a guest feedback cards for a restaurant?

nothing except a Subject.

In my experience, guest feedback cards in restaurants are printed cards
that can either be dropped at the cashier or mailed back, and thus have
return postage printed on them. I'd guess most of the restaurants simply go
to a local printing firm, tell them what they want printed on the form, and
order as many as they think they will need.

I don't see what this has to do with Microsoft Access database software, the
subject of this newsgroup. Perhaps you'd have a better chance of getting
your answer if you'd seek an appropriate newsgroup and clarify the question.

Probably using Microsoft Word to lay out a suitable appearing form;
print them out and provide them to the customers with a pen you don't
mind losing.

Or were you asking a question about Microsoft Access? Would you want
the customers to type answers into an Access Form?
