How do I get the "th" in "4th" and "5th" smaller?

Select it and press Ctrl+Shift+= or use Format | Font | Superscript.

You can set Word to autocorrect this whilst typing (from a field it is more
complicated) - tools > autocorrect options > autoformat as you type >
format ordinals with superscript.
