How do I get my Send & Receive button back?

My send & receive button has gona missing. I've tried the usual to get it
back - Tools, Customize, Commands, Tools, etc but to no avail. Does anyone
have any other suggestions?
 
You can not drag it from the Tools-->Customize-->Commands-->Tools selections
to the toolbar and drop it there?

Is one of the toolbars just not completely showing?

Is it there if you run outlook in safe mode or with the /nocustomize switch
from the run line?
