how do i enable the edit with frontpage button

After installing Frontpage 2003 the button on the IE menu is not available.
(edit with frontpage) How do you enable this button?
 
In FP
Tools - Options - General: Startup - unselect the last 2 "Check if .." options
In IE
Tools - Internets Options - Programs select FP as your default html editor

Mine are both checked, and I get the Edit button.

Lost the original thread.
In IE, right-click the toolbar and select customize. Add it from the
available toolbar buttons.
