HOW DO I DELETE AND "OLD" OUTLOOK CONTACT FORM?

HOW DO I DELETE AND "OLD" OUTLOOK CONTACT FORM? Employees getting confused
and using old form even though the correct form has a different name
 
Outlook version? Where is the form published? Where are the users seeing the
old form?
