How do I create new forms in Outlook 2003

I want to be able to create and publish my own forms in Outlook 2003. I can't
see a way to even start this process. Can anyone help, please

Design Form is located under Tools - Forms - Design a Form to start the
process.

This is to at least direct you on where to start.
