How do I create an Outlook Template for services downtime?

My project uses Outlook 2003. I'd like to have a "fill-in-the-blank" type of
template available when a service outtage (caused by me job) will occur.
Fill-in would be the date and hours only - names will come from Contact Group.

Please provide more information on how this template would be used and by whom. An Outlook form may not be an appropriate solution.

Sue, many THANKS for the quick response. The template I'm hoping to create
would be a general-broadcast tool to all users of the Configuration
Management tool that I'm the caretaker of. Within the body would be a
standard blurb stating that the server will be brought down from xx:xx AM
until xx:xx PM for software updates. The period of nonavailability would be
the only 'text' entered within the body.

So you'd be the sole sender? I'd probably handle this by saving an Outlook message as an .oft file with placeholders in the text for the times. You can then just run the .oft file and type in the dates or write a little macro to create the message and prompt you for them.

A published custom form definitely would not be a good solution for this scenario.
