How do I create a form that will submit responses to my email address?

I am having the dickens of a time creating a form that
when submitted will send the results to my email address.

I know how to design a form, however, I get the following
message after typing the my email address in the email
address field under "Where to store results" in Form
Properties: "This form is being created on a Disk-Based
Web or the FrontPage Server Extensions have not been
configured to send email. Please direct your system
administrator or ISP to the instructions in "Setting up E-
Mail Options on Windows" or "Setting Up E-Mail Options on
UNIX" in the Server Extensions Resource Kit. If you do
not have the Server Extensions Resource Kit, you can find
it at: http://www.microsoft.com/frontpage/wpp/serk."

I tried the latter URL but it must be out of date or no
longer valid, because I get "The page cannot be found
message."

Just for the record I am using FrontPage 2002 that I
purchased at Best Buy. My hosting service Avid Hosting
which does accept FrontPage Extensions for my website.
Why am I unable to create a the form (a survey), that
when completed and when submitted directs the results to
my email address?

Can anyone help, or where can I look for assistance?
Thank you!

You must have your host configure the extensions on the host server for
forms to email. You can't do it.

If you don't have a Web server running on your PC, just
treat this as a warning message that your form won't work
until you publish the Web site to your host (assuming that
as you say, the server extensions are installed and
properly configured on the host.)

If you want to test the form on your own PC, then you
would have to install a Web server and the Front Page
Server Extensions on your PC.
