How do I copy a designed form to another database?

I created a database for work done in 2004. Along with the database I
designed a form in which to enter the data I wanted to enter into the
database/tables. Now I want to created a new database for 2005, but I want
to use the form I designed for 2004 as the form for the 2005 database. I
tried copying the 2004 form to the 2005 database and it appears to work,
however when I open the form to enter data, the form window title at the top
says 2004 and I can not figure out how to change the title. Changing the
form header does not work. What can I do?
 
Are you sure you want to make a separate database for 2005. Normally you
keep all of your years in the same database and then use filters to deal
with 2004, or 2005 etc. This also makes it easier to generate historical
analyses.
