How can I print a telephone index?

Using Office Outlook 2003.
I wish to print a telephone index of my contacts for my small filofax.
I am able to print, via Excel, an index with all the telephone numbers for a
contact (Home, Bus, Mobile) across the page, in one line. This is too wide.
I have previously printed an index with only three columns - the first
containing the name, and the second has Home, Bus, Mobile etc under each
other, and the last with the corresponding numbers under each other - but
have forgotten how I did that.
Any advice will be appreciated

One way to do it is with a Word mailmerge. Start in your contacts folder and choose Tools | Mail Merge. The catalog format would fit your scenario.
