hit "reply" and change the "to" mail ID - outlook has no idea who.

This is with ref to Outlook.

You want to reply to a mail from X ...
Hit reply.
When the reply window pops up you decide you want to forward the mail to Y
and not reply to X. So you change the mail ID in the reply window and send.

Now the mail has gone to Y ......NOT to X.
Come back to the inbox - check the comment over the original mail from X. It
says "You have replied to this mail on ......." which is not true!!

so, you hit reply and then forward instead, and outlook is the one who has
"no idea?"
 
You use the Reply button to take an action on the message. That's what
Outlook recorded. Not a bug.
