help with obtaining approvals on one form

Looking for help and/or direction on how to set up a form to keep approvals
or denials on requests. I have created a form for end users to fill out
when asking for new account setups or when a change needs to be made to a
current account. Problem I have is that when I get the request I need to
send this form out to multiple data owners, in some cases, for an approve
or deny reply. currently what I do now if forward the form to each data
owner and wait for a reply, but this gets very cumbersome at times because
of the multiple replies. Ideally what would be perferred is to have the
form send to the approvers at one time and then end up in my mailbox with
the request and approvals all in one message form, or I get the request and
send out to the respective data owners and on a reply keep the approve or
deny in one message, similiar to a meeting request where you can see who
has accepted or rejected your meeting request. Also would like to show the
time stamp of the approval and the reply from to show the approval came
from approver a,b,c, etc... Any help would be appreciated.

If you use voting buttons for your approvals and send the message to all
approvers at the same time, the outgoing messages saved in your Sent Items
folder will contain a Tracking tab with information on who responded, how
they responded, and when.
