Custom Reply Form

I have created a mail message custom form. From the mail message custom form I created the reply custom form (the form that is sent when the supervisor approves or denies the request.)

The requester sends the time approval form which has the approve and deny buttons. Supervisor selects approve/deny and the reply form is sent
Problem:I can't figure out how to get the Approve/Deny buttons on the tool bar off of the reply form.

Check the (Actions) page of the reply form in design mode. That's where
custom actions are set up.
