help with contact manager unable to intialize form

When I look up a contact I get a message screen saying Contact Manager Unable
to Initialize Form. the general info page does not show but the details page
will. please help.
 
Hi

Check if Business Tools is avaliable else go to help > about microsoft
Outlook > Disabled items and enable BCM
