Header-and-Footer Toolbar

My Header/Footer Toolbar is not there anymore when I go into View:
Header/Footer and it is not docked with my other toolbars. How can I get it
back?

Have you tried going to view and then selecting header and footer? If you do
this, there is no reason why the toolbar does not appear.
