Header and Footer Toolbar has disappeared

When I view header and footer the toolbar does not appear (it used to!).
When I select Tools Customise Toolbars, it won't let me click in the box to
tick the Header and Footer Toolbar. I'm using Word 2000. How do I get it
back? - and how did I lose it?

If you can't select that toolbar even when you are in the header or footer
pane, then you will probably need to rebuild the Registry key that covers
toolbars. See
