Harddisk RIP?

My HD is 5yrs already & today it failed to start windows. Repair didn't work,
so I decided to reformat the disk.
At the end of the format screen (after 100%), the next screen said something
like 'windows unable to format the disk, the disk maybe damaged'.
Is this a sign that HD is dead?

Check with drive diagnostic tool of harddisk vendor.
