Getting screen images in my TOC !

I'm dealing with a protected document in Word 2003.

I'm writing a section that consists of a numbered list. Within each list
item is a sub-heading, then some text, then a table containg a screen image.

Some of the screen images are appearing in my table of contents. I don't
know how or why this happened or how to correct it.

Also, some of my pages are horizontal and some are vertical. How did this
happen and how do I fix this as well?

Those screen images probably were inadvertently formatted using a Heading
style. Click on the images and apply a body text style of some kind (such as
Normal, Body Text, etc., but not Heading 1, Heading 2, etc.).
