Getting data from a lost form

I have to format my disc and lose my contact form, who can I get all the data
for that form in an excel format. In order to make a new form without lossing
all the data in all the contacts.

Just save the form template as an OFT file and save your PST file (if that's
what you use). Then later on you can re-publish that OFT on the reformatted
disk and move your PST file there. You won't lose a thing.

Thank you Ken. My problem is that I lost the form when I formated my disk. I
only founded, in my backup, an .ofs (I don´t know the meaning or use of that
extension) file instead of an .oft file as you tell me. I have a complete
personal folders backup as a .pst file if that helps.

OFS is the file extension for Form Region storage (Outlook Form Storage).

If you have at least one instance of the form in existing items you could
use the Design This Form selection and then clear out all the data in all
the fields and republish.

If the form was previously published in a Personal Forms Library or folder
then recovering that PST file should retain the published form and you can
get it from that repository.
