Is your server a (1) Windows server and does it (2) support ASP?
Did you publish all required pages i.e. the database *.mdb file, to the
fpdb folder and the ASP pages to their relevant folders. Also did you
publish by FTP or HTTP?
In the form properties/options, are you sure you have the option point to
writing to the Access DAtabase and not to "other script" or to the
textfile/email?
