FP 2003 - how can i create a web form??

Anyone have any ideas as to how i can create a webform on my site?? i want it
to send the results to my email address.

Insert->Form and add form elements as required. Right click anywhere in
the form and choose form properties. Set up email and other options as
required. Requires FrontPage extensions on the server.

Alternatively, File->New choose More page templates (FP2003) and choose
a feedback form template. Modify to suit.
 
.....and then, make sure the host has properly configured the forms to email
transport.
