Forward Action Missing

I have a client that has a form that they have been using
for while. The person that had originally created the
form deleted the default actions from the Actions tab,
including Reply, Reply to All, and Forward. He replaced
them with Approve and Disapprove. The company now has a
need to modify the form and add the Forward action back
in. I can get it to add back in but it appears on the top
of the form along side the Approve and Disapprove
buttons. It works fine but the default Forward button in
the toolbar only forwards a copy of the form and they want
it to forward as an attachment. No matter what I do, I
cannot get the default actions back into the Actions tab.
I can create new ones that act the same but they do not
change the actions of the default ones. Does anyone know
how to get the default actions back in or is it even
possible? Thanks for the help.

What version of Outlook is this? I've been working with
another developer at my http://www.outlookcode.com site
who has OUtlook 2000 and a similar problem with the
Forward custom action disabled.
