Forms

I'm trying to add a name, phone & email address submittal form. When I go to
the 'Insert' menu. I choose 'Forms'; the option of 'List Forms' is grayed
out. What do I need to do to fix this? My FrontPage extensions are turned
on; I don't know what else to check. Help.

List Form, List Field and Form Web Part are used if your site is running
SharePoint Services. Select from the group of items under those choices, the
group beginning with Textbox.
