Hi,

I want to create a form that people can use to enter
data. I want this data to be added to a microsoft excel
list that I can use for various reports.

Can anyone suggest the easiest way to do this?

The easiest way is to select a cell in the list, and choose Data>Form.
For something a bit fancier, you could try John Walkenbach's Enhanced
Data Form. It's free, allows combo boxes, and has unlimited fields.
