Forms E-Mail Problem

Using FP 2003

I am trying to have form results sent to me via e-mail. I do not have any
form results posting to my site, only to e-mail.

However, whenever I try to submit my form, I get a pop-up window that
prompts me to enter the user name and password for my site. This is the
same user name and password box that comes up when I am opening my site from
FP.

Have you checked with your web host regarding using forms and/or this issue?

The page is not complete - it lacks </body> and </html> tags.

Also check with your host that they allow emails to be sent to addresses
not in your web site domain. The logon dialogue may be coming from the
SMTP server to authorise email relaying.
