Forms aren't working on site with Yahoo hosting

I have a client tha uses Yahoo small business hosting and wants a form for
their site. However, I can't get any thing to work. I have even tried to
use Yahoos' own form, but get an error:
Here is the site for any information for me.
http://www.iveysqh.com/contactus.htm

As I understand you can't use FP 2003 with their program. I made the site
in FP, then uploaded it by ftp. Just need to be able to use some kind of
basic form, to gather, name, address, phone numbers, etc. Any help would be
greatly appreciated.
thanks,
Lisa

FrontPage forms require FrontPage extensions on the server, and the pages
*must* be published using http:// not FTP.

There are no extensions on the server at the link you provided, therefore
the FrontPage forms won't work.

FTP probably killed the FP extensions.
What I would try is...
Have the extensions repaired.
Generate a new form with FP's form, not Yahoo's custom thingie.
Publish via FP's http:// method
Then see if the form works

As Steve said..there are no 2003 extensions. But, I had a web on Yahoo a
year or two ago, with FP extensions, and FP's forms worked fine.
