Format menu does not contain Paragraph.

"Paragraph" no longer shows on my Format menu, and does not always appear
when I right-click on text. How can I get Paragraph back on the Format menu?
 
Tools - Customize - Toolbars tab. Click on Menu Bar, then click Reset.

Or, if you have menu customizations you want to preserve, you can click on
the Command tab, set Categories to All commands, drag the FormatParagraph
command onto the Format menu (preferably where it was previously, although
this isn't essential).
