Form Error Received

I use FP 2003 with Server Extensions (Windows XP) and am creating a small
form (with only four fields: Name, Email Address, Phone and Description).
When I click "Submit", I receive this message:

FrontPage Error.
User: please report details to this site's webmaster.
Webmaster: please see the server's application event log for more details.

I left a message for my Host Provider to call me back, but does anyone know
why I'm getting this message? FPSE are enabled, but is there something else
that needs "enabling" or "checking" on my end in order for my forms to work?

I use FP 2003 with SE at work and my forms work. Of course, we have our own
server. This is the first time that I've dealt with a Host Provider. Thank
you for your help.

Your host must also configure the FrontPage Server Extensions to deliver the
e-mail to an e-mail transport - send them this link:
