Form created in Outlook will not send data filled in.

I created a form in Outlook. When someone opens the form and fills in fields
and selects check boxes then hits Send, the form appears in the correct
Inbox, but without any of the entered information, only the blank form itself.

Did you bind every control to an Outlook property (to store the data) and create a read layout that also has bound controls?

It's covered in the Help topics on Outlook forms, but I'm a firm believer in discovery through experimentation: In design mode, compare a control on the first page of the contact form with one of your controls that isn't doing what you want. Pay special attention to the settings at the top of the Value tab. You should quickly see what your controls may be missing.

Ah this is my problem too.

No offense Sue but some things may be more obvious to you than others. I
don't see anything specific to form controls under help.

Could you elaborate please?
