form and uploading files

i created a form that allows to upload the pictures , but there seems to be a
problem when i insert the upload file in the form it says choose destination
for file , it never used to ask before for this and when i publishe the site
and access form and fill in the form online and click sumbit, it than ask
for username and password, which i dont want to happen , because i didnt
apply any restriction on submission
and finally when i enter the username and password and succesfully submit,
the email hasnt attached a file but instead it give me a link to obtain te
file

please help

my front page template is a online template bought from website only HTML
format
the form data is collected by email

What are you using to process the form?
If using the FrontPage extensions, the uploaded file will not be
attached to any emails - it will be saved in the folder you designated
when you configured the upload form field (right click the form field,
and choose field properties).

If you use some other process for the form (PHP, ASP etc), then check
with the form handler's author.

The folder you upload to must have create/write permissions for the
anonymous user. Open the remote site and right click the folder in
FrontPage, then choose properties. Then clear the box "Allow scripts to
be run", tick the box "Allow anonymous uploads to this directory"
Click OK
