FLOPPY DRIVE NOT READING

INSERT DISK IN DRIVE A PROMPT IS ACTIVATED ANYTIME I TRY TO ACCESS DRIVE A.
DISK IS INSERTED. WOULD FILE SYSTEM NOT FOUND BE RESPONSIBLE IN SOME WAY.
 
Right-click on the A: drive in Windows Explorer and select Format - format
the disk. This procedure will erase everything on the disk.
