files did not defragment

I'm running Windows XP, C drive is extremely fragmented,
have run the defragmenter several times but still have
lots of fragmentation.

The report after the defrag runs lists many 'Files that
did not defragment'. How do I get these files fragmented
and why does not the defragmenter do this?
