File Association Type

I am trying to associate an extension with a program. I have tried by SHIFT-Right Click Open With but the ALWAYS USE THIS PROGRAM.... checkbox is greyed out. Under Tools / Folder Options / File Types as well the new and other buttons are greyed out. I noticed that this is the case on both of my servers running Terminal Services Application Mode (even from the console), but from my other servers running Remote Mode these buttons are available. Windows 2000. Anyone have any idea how to get around this. All Service packs and updates have been applied.
