faxing scanned images with a cover page

When I scan a document and then send it via the print
function to Microsoft XP Fax I get the fax wizard window.
I then put in the necessary information and here is what
happens: if I choose no cover page my scanned image(s)
will fax correctly, if I select a cover page I get a
window saying the wizard is finished and then nothing
happens. My faxes don't go out and I cant find them.
They are not in the outbox of the Fax console. What is
happening and how do I send scanned images with a cover
page?

When you select a cover page, in the final dialog in the send fax wizard,
when you preview the fax are you able to see it correctly?
What scanner are you using?
