Fax cover page problem

I have no problem sending faxes from applications. But as
soon as I attach a cover page and finish the fax does not
go, and all the info that I have typed on the note is lost.
any Ideas
 
Post the fax software you are using, its configuration, and the steps you
are using to send the fax. You haven't even provided a hint.
