Failure to hide icon in tool bar. . .

All,
Even though I have the radio button checked for "Hide
inactive icons" and the icons have "Hide when inactive"
status under the custom button. Also, the ">>" button does
not show, and I know I have at least one icon or two that
are not active. Do anyone have a clue what I should try?
