Extending "Save As" Dialog

My question is:

How can i extend the "Save As" Dialog. For example if i
am working in Word and i want to save my document in my
new app, i want to select "save as" and i want my app to
be shown in the options.

I hope to be clear.

Sorry, I don't have .NET code to do that but basically it's possible
with "dialog templates". You will find a VB6 implementation here:
