Exporting .cer to .pfx

As per this KB article, I've been trying to export a .cer file to .pfx format
and am having trouble.

In step 5 of the Export procedure, it lists a step to select Personal
Information Exchange as the file format for the export. Unfortunately, that
option is greyed out for me. I have the other 3 options (.cer, 64-bit .cer
and .P7B). The box I created the cert request on was Windows NT and doesn't
have a certificates snap-in, so I've been trying to do the export on another
(Windows 2000) server, with no luck.

Any suggestions?
