Error when summitting a form

Hi

We have a free credit form on our web site, when it is
completed and summitted, there is an error stating

URL:
http://domain.com/_vti_bin/shtml.exe/creditapplication2.ht
m
Error Message:
FrontPage Error.
User: please report details to this site's webmaster.

Webmaster: please see the server's system log for more
details.

ISP states it's something on the customer's end.

domain is hosted by an ISP
Using FrontPage 2002 on Win ME

any ideas to what can be the cause?

What does the system log say? (The error is too generic for any sort of
diagnosis.)
