error loading form. some of the needed controls may not be registered

I just installed BCM, i think version 2. When i launched outlook 2003 pro after installation of BCM, the system went thru the installation and association of DB. However, when i try to create any of the items, i.e. Business Contact, Account, oppertunity, etc., i get an error:

error loading form. some of the needed controls may not be registered

I have been searching the net for past couple of hours and have not found any solution to this. Do any of you have seen this error, if yes, what is causing it and how can I resolve it.

We have Exchange 2003 and i am using outlook 2003 as exchange client.
