Envelope is blank after printing

I am using Office (Word 2008) version 12.1.2 for Mac and was able to print
envelopes without a problem until about a week ago. I have not changed any
settings. The mailing address shows up on the envelope when I type it in, but
in the print preview it is blank, and when I print, it is blank, though the
envelope goes through the printer.

I tried inserting a normal sheet of paper to see if the margin settings were
off, but the piece of paper was blank also.
