Embedded Icon on File, Print, etc, toolbar

I discontinued an internet provider that imbedded their
icon on this toolbar. I downloaded and installed IE 6.0
over the 5. version, but it is still there. How do I get
it off?

Or... an easier way which should also work:
To remove branding from IE:
Start> Run> type in or copy and paste:

Rundll32 Iedkcs32.dll, BrandCleanInstallStubs

Click OK.
Restart Internet Explorer

Using ToolbarCop to remove the unwanted Toolband, Toolbar Icons and BHO:

Look at the entries one by one in the first section "Internet Explorer Toolbar Buttons", the items under the Tools menu will be listed here. Same for Toolbar icons.
