email with attachments being sent multiple times

A friend using Outlook seems to have a problem where there is an attachment -
photograph or a document - where some recipients receive multiple copies of
the same email. Sometimes it appears the message has not been sent
successfully when indeed recipient's inboxes become full. Could this be
caused by a virus, an incorrect setting? Any solutions?
 
Without any information about his/her configuration setup (Outlook version,
account type, etc) it is hard to give you an answer. You could definitely
rule out a virus though.
