Unable to get my email form to work. Front Page extensions are enabled. Any
suggestions?

Define: "Unble to get my email form to work." What precise steps did you and
your host take to set it up? What errors are you getting?

I followed the directions in FP and set it up accordingly. And uploaded to my server via FTP. Following is the message I get when hitting "submit"

FTPing a FrontPage form will always cause disaster.
FrontPage forms use the FrontPage extensions, and using FTP will:

1) not allow the extensions to modify the form code to suit the server type
and configuration

2) Corrupt the extensions

The extensions will probably have to be repaired or reinstalled due to your
use of FTP, and you must NEVER use FTP to publish to the website - Use HTTP
method from the FrontPage File->Publish menu.
